Grant Fund Recipient Report

Use this form to record the results of the grant money you received.
  • (a) Describe the progress made toward the goals and objectives as stated in your funded grant application. (b) Summarize the key data measurements related to the funded grant.
  • Describe the significant successes and challenges you experienced related to the funded grant.
  • a. Describe what was learned based upon the results, successes, and challenges reported in Questions 1 and b. Address programmatic, evaluative, or other changes that will be made based upon these lessons learned.
  • Share anything else that happened during the grant period that impacted your student, either positively or negatively.
  • Submit the following attachments along with the narrative report. Label each attachment. 1. FINANCIAL STATEMENTS Include a budget of your expenditures based on the grant together with receipts. 2. PHOTOS AND OTHER STUDENT INFORMATION Include any photos of your projects, students, equipment, etc. that were a result of the grant. Be sure to have parental consent to release all photos.
